Primary application form

Applications must be submitted at least eight weeks in advance and must include all relevant supporting documents.

ثانوي

To request special leave for a student attending OIA Secondary, please download and complete the form below.

The completed form should be printed and submitted at the reception of Haarlemmermeer Lyceum Tweetalig, Baron de Coubertinlaan 2.

Planned requests should be submitted as early as possible and at least two weeks in advance. Applications for holiday leave outside the school holidays must be submitted at least eight weeks in advance.
